A SPECIAL FITNESS CENTER REQUEST FROM THE MANAGEMENT & STAFF:

Per our Fitness Center policy, please wipe off all equipment and machines after usage with the materials provided in the room. This insures that we are maintaining our equipment and providing a clean and proper environment for all our fitness center patrons. This includes all trainers using our center for training of our residents.

Thank you for your cooperation and assistance.

FITNESS CLASSES

PLEASE NOTE IMPORTANT CHANGES TO THE FITNESS PUNCH CARD SYSTEM:

Due to the declining number of participants in our current fitness classes, we have been subsidizing a considerable amount of the fitness instructors’ payments since the inception of the Punch Card System in July. Although this new system has been received very favorably by participants, the number of participants is not funding the instructor payments. This subsidization, paid from previous class profits or the “Fitness Fund,” is almost depleted and it is therefore necessary to increase the cost of the Punch Cards.

EFFECTIVE FEBRUARY 1, 2012, THE COST OF THE PUNCH CARDS WILL BE $45.00 FOR A 10 CLASS CARD.

If we approach a point when the subsidization of the fitness class program is decreasing, we will re-address the cost of the Punch Cards and return to the lower cost.

In addition, the current class schedule was reviewed and the following classes that have extremely low participation will be suspended from the schedule, beginning in February.

DISCONTINUED FITNESS CLASSES

If you have suggestions or comments about this issue,

please contact Tracy Citrano or Erica Horndasch.

Your support and input is important, valued and very much appreciated.

Thank you for your consideration.

HELPING HANDS

Do YOU need a helping hand? Maybe Helping Hands can help!

Helping Hands is a group of residents who want to lend their “hands” to other residents within the community who may need assistance of some kind. This may take the form of:

· A social phone call or get well wish

· Help around the home in time of sickness or healing

· A ride to an appointment, a store, pharmacy, physical therapy on a limited or occasional basis

· A wellness visit

· A hospitality visit

· Card recognition for illness or loss of a loved one

· Community information dissemination

· Local resource materials

· Web portal training

The group’s purpose and function is based solely on the need for assistance and will maintain the utmost sense of privacy and respect at all times. The members will rely on requests by residents ONLY and all assistance will be given on a confidential basis unless otherwise requested by a resident.

If you are in need of assistance by the group or have permission by someone to make a request on their behalf, please contact the person in Helping Hands from your neighborhood listed below. MUNDELEIN POLICE DEPT. HOUSE WATCH PROGRAM

As a service to our community, the Mundelein Police Department will do security patrols for the homes of residents who notify the department that they will be away from their homes for extended periods of time. This would include more than one day periods and depending on time constraints, would include daily drive by monitoring and walks around the home. If you are interested in this service or would like more information,
please notify the Mundelein Police Department at the 24-hour, 7 day a week, Non-Emergency number, 847-968-4600.

"FILE FOR LIFE"
The "File For Life" program sponsored by the Mundelein Fire Department, helps the fire department "help you" by making your medical information available to the responding Firefighter/Paramedics in a medical emergency at your home. Forms are available near the bulletin board area at the Lakeside Lodge. Completed forms should be kept in a visual position on your refrigerator, making it accessible for emergency personnel.
If you need further assistance, or have questions, contact the fire department at 847-949-3260 or www.mundeleinfire.org

TAX EXEMPTION INFORMATION

For those residents who are 65 years of age and older, you can file for a Senior Tax Exemption at the Fremont Township Tax Assessor’s Office. Forms can be picked up at 22376 W. Erhart Rd., Mundelein, 847-223-2846.

LAKESIDE LODGE ACCESS CARDS

Residents must now use their access cards for entry into Lakeside Lodge during hours of operation. Please remember to bring your card to enter the Lodge. This is imperative if we are to properly monitor visitors’ entry and it is an important aspect of the security measures put into place for the Lodge. Access cards are issued after participation in a fitness center orientation session. If you do not have an access card, please sign up for an orientation session. To schedule an orientation session, contact the Lifestyle Director, Tracy Citrano, to set up a time, 847-566-2403. If you have gone through an orientation session and have not received your card, please inquire at the Lakeside Lodge front desk.

Lost card fee replacement is $25.00.

ACCESS CARDS ARE NON-TRANSFERABLE.

ACCESS/ACTIVITY CARD CLARIFICATION

While all Grand Dominion homeowners are authorized facility use privileges, a maximum of two access/activity cards are issued for each Annual Base Assessment (as noted in the Governing Documents.) This does not mean a single resident household will be issued a second card if there is not a second authorized resident in that home. Additional residents over the two person card maximum, 19 years of age or older, who reside in a residential home for more than 6 months in a calendar year, may obtain a supplemental annual Access/Activity card at an annual cost of $500.00.
