FITNESS CLASSES
PLEASE READ THE FOLLOWING FOR
IMPORTANT CHANGES TO THE GRAND DOMINION FITNESS CLASS PROGRAM
In an effort to increase our fitness class participation and provide the best possible and affordable fitness class program, our Fitness Class Registration Procedure will be undergoing some changes. On July 1st, we implemented a
FITNESS CLASS PUNCH CARD SYSTEM.
Please note the following:
THE FITNESS CLASS PUNCH CARD SYSTEM WORKS AS FOLLOWS:
Ø Cards will be sold at the front desk - $40.00 for 10 classes (or punches).
Ø Punch cards can be used for any class type and combination with the exception of the S.T.A.B.L.E. Balance Classes.
Ø The fees for the S.T.A.B.L.E. Balance classes will remain at $1.00 per class and that fee will be paid directly to the instructors from Grayslake Rehabilitation at the beginning of the class. No pre-registration will be necessary.
Ø Punch cards will not be used for the S.T.A.B.L.E. classes.
Ø Cards are non-transferable and can be used by only one resident.
Ø At this time, expiration dates are not applicable.
Ø Residents without a punch card can attend any class for a $5.00 drop-in fee.
Ø No Pre-Registration will be necessary for residents using a punch card or paying the $5 drop-in fee.
Ø The Front desk will no longer take any money or handle any registration – Registration will be done by the instructors in each class - they will take attendance, punch the cards and take any money for the drop-in fee of $5.
Ø Classes will be held regardless of number of participants. Classes will be held as scheduled unless advance notification has been made or emergency purposes dictate cancellation.
Ø This system is being implemented on a trial basis, will be evaluated at the end of the year and is subject to changes at that time if continued subsidization is necessary to fund instructor fees.
For more information other inquiries,
please contact the Lifestyle Director, Tracy Citrano.
We hope the new system will be equally beneficial for all involved.
Thank for your patience and cooperation during this transition period.

please notify the Mundelein Police Department at the 24-hour, 7 day a week, Non-Emergency number, 847-968-4600.
"FILE FOR LIFE"
The "File For Life" program sponsored by the Mundelein Fire Department, helps the fire department "help you" by making your medical information available to the responding Firefighter/Paramedics in a medical emergency at your home. Forms are available near the bulletin board area at the Lakeside Lodge. Completed forms should be kept in a visual position on your refrigerator, making it accessible for emergency personnel.
If you need further assistance, or have questions, contact the fire department at 847-949-3260 or www.mundeleinfire.org
